Before your consultation
For a conservatorship
The person's diagnosis or condition, current living situation, any powers of attorney or health care directives, and the names of close relatives.
For a limited conservatorship
Your child's date of birth, regional center and IPP information, recent IEP, and who will serve as conservator.
For probate or a trust
The death certificate, any will or trust, and a rough list of assets and how each is titled.
For a guardianship
The child's birth certificate, information about the parents and why they cannot provide care, and the child's school and medical information.
